Massage Therapy: It’s Not Just for Rest and Relaxation!

What do you think about when you hear the word “massage”? If you’re like many people, you associate massage with a day of pampering at an exclusive resort spa in the mountains or at the beach. But if that’s the first picture that comes to mind, you might be missing something very, very important. That something is the therapeutic value of massage—the ways that massage can actually improve your health and well-being.

While it is certainly true that many types of massage do help with relaxation, therapeutic massage also has a variety of important health benefits. For instance, therapeutic massage:

  • Accelerates healing by improving the circulation of blood and lymph to injured areas
  • Promotes flexibility by stretching and loosening muscles and connective tissue
  • Improves muscle tone and helps prevent or delay muscle atrophy cause by prolonged periods of forced inactivity
  • Relieves pain in joints by reducing inflammation and swelling in joints
  • Increases the effectiveness of chiropractic adjustments

Practitioners and patients alike have discussed these types of benefits for many years, but researchers have recently made a great deal of progress collecting and analyzing clinical data to understand the effects of therapeutic massage. Here are some “headlines” from their work as well as a few “notable quotes” from their study findings.

Improved Circulation and Post-Exercise Pain Relief

Investigators at the University of Illinois at Chicago recently conducted a study to determine whether or not massage improves general circulation and relieves soreness after exercise.

  • “Our study validates the value of massage in exercise and injury, which has been previously recognized but based on minimal data,” said Nina Cherie Franklin, UIC postdoctoral fellow in physical therapy and first author of the study. “It also suggests the value of massage outside of the context of exercise.”
  • “We believe that massage is really changing physiology in a positive way,” said Franklin. “This is not just blood flow speeds—this is actually a vascular response.”
  • Because vascular function was changed at a distance from both the site of injury and the massage, the finding suggests a “systemic rather than just a local response,” she said.

Reduced Chronic Low Back Pain and Improved Mobility

There are more than 100 million massage therapy visits in the U.S. each year, and lower back pain accounts for more than one-third of them. Why?  Because massage works!

A study in the Annals of Internal Medicine found that patients receiving massage (whether “structural” massage or “relaxation” massage) had better outcomes than those receiving typical medical care without massage. Measured after 10 weeks and again after 26 weeks, patients who received massage had less pain and better mobility than the control group. At 52 weeks, the results were less clear.

A review in Harvard Health Publications contained some very positive words about the study, which was funded by the National Institutes of Health’s National Center for Complementary and Alternative Medicine:

  • “My interpretation of this well-designed study is that massage appears to be at least as effective as standard treatments for chronic low back pain.  It might even be better.  And it’s likely safer than medicine.  Depending on actual costs and insurance coverage, massage may even cost less than usual care.”
  • “In my opinion, it’s time massage became a more standard option for the treatment of chronic low back pain.”

The Chiropractic Care – Massage Therapy Connection

Massage is often recommended as an integral part of a broader chiropractic treatment plan, either in the form of massage to loosen muscle tightness before an adjustment, or after an adjustment to help the muscles adapt to the newly-repaired spinal structures. Many patients report that the combination of these two therapies works better than either therapy alone. In our experience, this is most noticeable with conditions that cause chronic pain—chiropractic manipulation or mobilization techniques work to relieve the structural problems and therapeutic massage works to resolve the soft tissue problems. This is why many chiropractors work closely with massage therapists to find the most effective treatment regimen for each patient—the one that returns them to a feeling of health and well-being as quickly as possible. The Most Dangerous Jobs: A Chiropractic Perspective

It’s no secret that some jobs are just more physically demanding and more dangerous than others. Some of the most challenging ones are obvious—combat roles in the military, fire and rescue, heavy construction and deep sea fishing are just a few that come to mind. However, when it comes to non-fatal musculoskeletal injuries, the statistics tell a very different story about occupational health and safety and about who’s most likely to get hurt on the job.

Because chiropractic physicians are experts in treating conditions that involve the musculoskeletal and nervous systems (including many common workplace injuries), we have a unique perspective on why they happen, how they can  be prevented and the best ways to treat them.

Musculoskeletal Disorders by the Numbers

Let’s start with a long-term trend that’s good news for the nation’s workers but that doesn’t get much media attention. The number of reportable occupational injuries and illnesses has declined steadily across the past decade from 50 cases per 1,000 full-time workers in 2003 to 33 cases in 2013. So progress is clearly being made, even if it doesn’t grab the headlines.

That said, musculoskeletal injuries continue to be among the most common on-the-job injuries, and they can require significant recuperation time. The Bureau of Labor Statistics (BLS) says that musculoskeletal disorders (MSD) “are injuries or illnesses affecting the connective tissues of the body such as muscles, nerves, tendons, joints, cartilage, or spinal disks.”

A very high percentage—about one-third—of 2013 workplace injury and illness cases that required days off from work involved the musculoskeletal system.  Plus, workers who sustained musculoskeletal injuries required more time to recover before returning to work—a median of 11 days to recuperate compared with 8 days for all types of workplace injury and illness cases. In 2011:

  • The back was the primary site of MSD injuries in 42% of all cases across all occupations, requiring a median time off of 7 days to recuperate.
  • Although it accounts for only 13% of all MSDs, the shoulder was the area with the most severe injuries, requiring a median of 21 days off of work to recuperate.
  • Injuries and illnesses due to repetitive motion involving “micro-tasks” (such as typing) accounted for only 3% of all occupational injury and illness cases. However, those workers with this kind of injury required nearly 3 times as many days away from work to recover as those with all other types of injuries and illnesses—a median of 23 days.

 

As we mentioned earlier, though, MSDs are not distributed evenly across all industries and occupations.

  • In 2013, the highest MSD incident rates were found in transportation and warehousing (80.3 cases per 10,000 full-time workers), healthcare and social assistance (50.2 cases per 10,000 full-time workers), arts, entertainment and recreation (46.5 cases per 10,000 full-time workers) and construction (41.9 cases per 10,000 full-time workers).
  • In 2011, six occupations accounted for 26% of all MSD cases: nursing assistants, laborers, janitors and cleaners, heavy and tractor-trailer truck drivers, registered nurses and stock clerks.
  • In 2011, those with the greatest number of median days spent off from work in order to recuperate from an MSD were heavy and tractor-trailer truck drivers (21 days).

So it’s clear from the numbers that the dangerous jobs featured on reality TV shows (think about Deadliest Catch and Ice Road Truckers) aren’t necessarily the ones that result in the most MSD-related visits to the chiropractor or general practitioner.

What is “Referred Pain”?

What is “Referred Pain”?

Billings Chiropractor“Referred pain” can be a perplexing phenomenon for anyone who experiences it. Referred pain is what happens when you feel pain in an area of your body that is not actually the original source of the pain signals. The most common example of referred pain is when pain is felt in the left arm, neck or jaw of a person suffering a heart attack, while they often have no feelings of pain in the chest area itself.

It’s important to note that referred pain is different from radiating pain, in which the pain felt in one area travels down a nerve, causing pain along the length of the nerve. This is often the case with sciatica, where pain originates in the lower back and radiates down the leg.

Researchers are still not exactly sure what causes referred pain. Some experts believe that it is due to a mix-up in nerve messaging. The central nervous system (CNS) is constantly receiving a barrage of different messages from different parts of the body. These messages may get mixed up somewhere along the path between the place where the irritated nerve is signaling and the spinal cord or brain where pain signals are processed. With an extensive network of interconnected sensory nerves that serve the same region of the body, such as the nerves of the lower back, thighs and hips, it may be more common for signals to get mixed up than you might imagine.

Although referred pain is usually felt as painful, it can also cause feelings of numbness, tingling or the sensation of pins and needles. Another example of referred pain is a tension headache, in which headache pain is due to an irritation of the nerves in the neck.

Referred pain tends not to cross sides of the body. In other words, if the pain signals are originating in the liver or gallbladder (which are on the right side of the body), you may feel pain in your right shoulder. If the signals originate in the pancreas (on the left of the body), you may feel pain in your left shoulder, etc.

Chiropractic adjustments can address the source of the referred pain, leading to long-term pain relief. Nerves in the area of the spinal cord that are irritated due to a spinal misalignment (subluxation) can be a cause of referred pain. When your chiropractor adjusts your spine, he or she removes the source of irritation, thus providing relief. It’s no accident that so many philosophers and writers have used the backbone as a metaphor for discipline, force of will or character.  Your spine (or “backbone”) is the primary physical support for your body’s entire frame.  It’s a remarkable piece of natural engineering composed of 33 separate vertebrae that act as a single unit to provide stability as well as flexibility while you’re sitting, standing or in motion.  A healthy spine is both strong and resilient.  With proper nutrition, exercise, postural habits and chiropractic care, it can allow us to lead an active lifestyle well into old age.  However, poor biomechanics, injury and disease can cause problems with the spine that result in misalignment, inflammation, pain and restricted movement.

The spine develops from infancy into adulthood, gradually evolving from a C-shape, which is suitable for crawling, to its distinctive S-shape, which is the appropriate shape for two-legged walking.  The natural curves in the spine serve to cushion impact from movement, absorb shock, preserve balance, and allow range of motion.  The three main curves in the spine are known as the cervical curve (the neck region), the thoracic curve (the upper back) and the lumbar curve (the lower back).

There are 7 cervical vertebrae, 12 thoracic, 5 lumbar, 5 sacral and 4 coccygeal vertebrae.  The sacral and coccygeal vertebrae are those located lowest in the spine, below the area of the lumbar curve.  Twenty-four of the vertebrae in your spine are moveable. They are cushioned by intervertebral discs which act like coiled springs. These discs are fluid filled and—as we age—become thinner and more brittle, often causing us to get shorter.  Over time (or as a result of excessive wear and tear or specific injury), they can degenerate, bulge or herniate, potentially causing significant pain and loss of mobility.

There are several other common spinal disorders.  Lordosis, also known as “sway back,” occurs when there is abnormal forward curvature of the lumbar spine.  Those who have abnormal curvature of the thoracic curve have kyphosis, or “hunchback.”  Scoliosis occurs when there is a side-to-side curvature in the spine. Slight curves of less than 20 degrees do not usually present health or problems and are seldom treated.  Moderate or severe curves usually require treatment because they can interfere with the functioning of internal organs and may significantly limit physical activity.

While the bones and connective tissues of the spine are very good at what they do, they cannot support the body’s weight and facilitate movement on their own.  They need the help of strong core muscles.  Good muscle tone is important to help maintain proper posture and spinal alignment.  This is why it’s so important for us to put effort into maintaining proper posture when we sit, stand, lie, walk and run.  Over time, poor posture can place unnatural stresses on the musculoskeletal system (especially the spine), limiting our range of motion and producing pain.

In addition to its role in supporting the body’s frame and facilitating movement, the spine has another important role as well.  The spine’s bony vertebrae also encase and protect the spinal cord, which is connected directly to the brainstem.  It’s hard to exaggerate how important this protection is.  Damage to the spinal cord can cause numbness and loss of motor function.  Injury to the cervical area can cause tetraplegia (also known as quadriplegia), while injury to the thoracic or lumbar area may result in paraplegia, or loss of the use of the legs and trunk.

back-pain-office-200-300About a third of the millions of people who make appointments with chiropractors every year seek relief from back pain. Back pain can be acute, meaning it happens suddenly, lasts 6 weeks or less and often clears up on its own; or back pain can be chronic, meaning it comes on gradually and lasts 3 months or more. Chronic back pain can be particularly debilitating and can limit movement and mobility.

Traditional treatments for back pain include medication, physical therapy, surgery or steroid injections. While these treatments may provide symptomatic relief, they do not address the root cause of the pain. They can also be painful and expensive to carry out.

The foundation of chiropractic care for chronic back pain is the understanding that misaligned vertebrae can cause the pain. This misalignment can result in many additional problems, such as headaches, body pains and impaired joint mobility. Chiropractic treatment aims to restore alignment to the vertebrae, returning natural health to the spine and all the body parts the spinal nerves serve.

Chiropractors believe in the body’s natural ability to heal itself. Chiropractic care avoids medications and their possible side effects, and it also avoids surgery. As an example of the differences in treatment, surgeons may remove a herniated disk from the spine in order to relieve pressure on the nerves, while chiropractors use non-invasive spinal manipulation to achieve the same result.

Dr. Oblander can treat your chronic pain based on the vertebral misalignments found in your body. A quick, sudden force is applied to the appropriate vertebrae in order to restore the motion of the joint. Another common treatment for chronic pain is known as the flexion-distraction technique. This treatment involves a special table that stretches the spine. It is particularly effective in treating injuries to the discs that have been the cause of long-term back pain.

Chronic back pain will probably also require additional treatments such as massage, exercise, and perhaps physical therapy. A good chiropractor will work with other health professionals as needed to ensure you get the best possible treatment for your pain. He or she will also look at the entire picture of your life, including your diet, health habits, medical history, family history, and other conditions you may have. This approach is holistic and has a better chance of eliminating the root cause of your chronic back pain than traditional treatments that only work on the symptoms.

Good News About the Cost of Chiropractic Care

A recent evidence-based report on the effectiveness of chiropractic treatment for people dealing with low back and neck pain contains some very good news. And it’s good news for three important groups of people—patients who benefit from chiropractic, employers who sponsor health care plans, and insurance companies who manage the costs.

The report is called “Do Chiropractic Physician Services for Treatment of Low Back and Neck Pain Improve the Value of Health Benefit Plans?”. It was prepared for the Foundation for Chiropractic Progress by a panel of esteemed medical authors who were tasked with investigating the clinical effectiveness and economic efficiency of today’s chiropractic care.

In their summary, the authors concluded that chiropractic care is “more effective than other modalities in treating low back pain and neck pain.” In terms of cost, they determined that chiropractic care for low back pain, compared with medical physician care, increased annual spending per patient by only $75 per year. For neck pain, there was actually a cost savings – the cost of chiropractic care compared with medical physician care was reduced by $302 per year.

In terms of overall cost-effectiveness, the authors of the report concluded: “When considering effectiveness and cost together, chiropractic physician care for low back and neck pain is highly cost-effective, represents a good value in comparison to medical physician care and to widely accepted cost-effectiveness thresholds.” They also concluded that their cost-savings estimates were likely to be “understated” since their analysis didn’t capture reduced spending on over-the-counter and prescription medications that would have been associated with conventional treatment.

This should indeed be seen as good news by the many Americans who already use chiropractic care to treat their back and neck pain. It should also be considered good news for American employers and health care providers concerned about providing the best value for every dollar in their health benefit plans. But the good news isn’t limited to the US—the researchers found the effectiveness and cost figures also applied in the European Union.

The report authors concluded with an even stronger statement in favor of chiropractic care: “Our findings in combination with existing US studies published in peer-reviewed scientific journals suggest that chiropractic care for the treatment of low back and neck pain is likely to achieve equal or better health outcomes at a cost that compares very favorably to most therapies that are routinely covered in US health benefit plans. As a result, the addition of chiropractic coverage for the treatment of low back and neck pain at prices typically payable in US employer-sponsored health benefit plans will likely increase value-for-dollar by improving clinical outcomes and either reducing total spending (neck pain) or increasing total spending (low back pain) by a smaller percentage than clinical outcomes improve.”

Good posture isn’t exactly a high priority for many Americans. For millions of us, the number-one priority is working to provide for our families—and sitting all day at a desk is how we achieve that. However, poor posture while sitting at work for many hours every day can actually lead to poor posture while standing the rest of the time—and that’s a more serious problem than one might think.

A Wall Street Journal article entitled “How Bad Sitting Posture at Work Leads to Bad Standing Posture All the Time” talks at length about this phenomenon.  Allston Stubbs, an orthopedic surgeon at Wake Forest Baptist Medical Center who treats patients with back or joint pain, puts it this way: “[Posture] is probably the 800-pound gorilla when it comes to health and wellness…  We see the spine and overall skeletal structure being critical to a patient’s functionality and their satisfaction with their life and health care.”

This means that many Americans’ habit of sitting all day with no thought to their posture has severe consequences—without good posture, many people can develop serious neck, shoulder, and back pain, leading to a sharp decline in their quality of life. Sitting all day with poor posture can lead to muscular back pain, herniated discs, and even pinched back nerves.

Poor sitting posture can also cause tension headaches, diminished breathing, and fatigue. It can even make you look older, according to the LA Sentinel. “Never underestimate the beauty and health benefits of good posture. Often poor posture is just a bad habit that is easily corrected. Poor posture not only makes you look older, but could be the first step toward dowager’s hump, double chin, potbelly, and swayback as well as some internal problems too. When a person is hunched over or not standing straight, that person may be perceived as older than they actually are. Good posture is not only beneficial to your body; it also makes you look taller and slimmer. What’s more, good posture can convey self-confidence, which may just be the best accessory you can have.”

Additionally, good posture is essential for a healthy spine. It can reduce or eliminate back (and shoulder and neck) pain, and it can even improve your mood.

However, there are millions of people today who simply have not learned what good posture is—and it’s not standing rigid, with shoulders thrown back, as many may have learned in childhood. Rather, as the WSJ articles says, “Good posture doesn’t just mean standing with the shoulders thrown back. More important is maintaining good alignment, with ears over the shoulders, shoulders over hips, and hips over the knees and ankles. Body weight should be distributed evenly between the feet.”

While workplace-related posture problems are getting a lot of attention in the media these days, the importance of good sitting posture to office workers’ health is hardly news to the U.S. government. The United States’ Occupational Safety & Health Administration (OSHA) offers a number of tips for good sitting posture, including keeping your head in line with your torso as well as keeping your elbows close to your body and your thighs and hips parallel to the floor. OSHA also recommends using a well-padded seat, keeping your shoulders relaxed, and making sure your forearms, wrists and hands are straight and well-aligned.

People in the US who are interested in staying in shape tend to favor running or working out at the gym to maintain fitness. But how do people in other parts of the world stay fit? And what might Americans learn from them? Following is a summary of how people in some other countries keep in shape.

The Netherlands and Scandinavia – The number one form of physical exercise in these countries is undoubtedly the bicycle. They use it to do everything from getting to and from work and school to shopping for groceries and cycling someone home from a date. There are dedicated bike lanes in every city, and many that stretch from one side of the country to the other. From small children to the elderly, everyone cycles as a matter of course, and their overall fitness shows how beneficial it is to their health.

China – You may have seen people in public parks around any major city (or any community with Chinese immigrants) performing a series slow, precise movements. Tai Chi is an ancient Chinese form of martial arts that is good for conditioning not only the body, but the mind as well. It is especially good for seniors, as it improves balance and overall mobility, increases strength in the legs and reduces stress.

India – Most people are surprised to learn that the physical practice of yoga as we know it is not actually ancient. Yoga in India was traditionally a mental and spiritual practice that incorporated special breathing techniques while in a sitting or standing posture. The type of exercise that comes to mind when we think of the word “yoga” (incorporating different postures called asanas, in combination with measured breathing) was not developed in India until the early 20th century. Nevertheless, yoga is now practiced very widely in India and all over the world as a way to promote overall mind-body fitness. It strengthens and stretches the muscles, increases flexibility and reduces stress.

Japan ­– Radio Taiso is a program of simple calisthenics set to music that is broadcast several times a day and is used by schools and many companies to help keep people fit. There are three official versions. One is a general fitness program for people of all ages, one is specifically designed to increase physical strength, and one is geared toward the elderly and disabled, who can perform the exercises while sitting down.

Brazil – An energetic form of dance that is taking off elsewhere in the world as well, Capoeira is a combination of dance, martial arts, acrobatics and music. It improves balance, coordination and rhythm while strengthening all the muscle groups. It will also improve your reflexes and help increase concentration.

Middle East – Belly dancing was originally developed as an exercise to aid women in childbirth. But belly dancing has many additional advantages. It particularly targets the “core” muscles, which are the ones responsible for keeping us upright and which have all too often become weakened by sitting for long hours at a desk. By strengthening your core muscles, you reduce your risk of suffering a back injury or chronic lower back pain.

You know the big things that impact your posture, such as the height of your keyboard or whether you slouch on the couch, but it’s easy to overlook the little things. By the way, where is your wallet right now? If it’s currently in your back pocket, we need to have a talk…

Little Things Matter When it Comes to Posture

Your wallet fits so perfectly in your back pocket. Certainly it can’t hurt to keep it there, right? Unfortunately, sitting on your wallet can cause a host of posture problems, which can lead to pain in your back, shoulders, and neck. When half of your posterior is higher than the other, your pelvis twists, the spine becomes misaligned, and your shoulders have a tendency to slump. This isn’t good, but there is a simple solution: just keep your wallet in the front!

Now that your wallet is in the right place, it’s time to look at your feet. What kind of shoes are you wearing? If you’re a woman wearing high heels, think about giving your back a break. Tall heels put you off balance, which your body compensates for by flexing at the hips and spine. The forward curve in your lower back decreases, your knees are stressed, and the muscles in your back, hips, and calves tense. All of this can lead to poor posture and back pain. Switch to flats to solve the problem.

Guys, you aren’t off the hook in the footwear department. If it’s been a while since you’ve bought a new pair of shoes, take a closer look at your soles. Wear and tear in this area can throw off your gait, leading to posture issues and—you guessed it—back pain. Maybe it’s time for some replacements…

Women with large breasts might find it particularly difficult to maintain proper posture. This is more likely to be true without the support of a proper bra. If this situation is causing you to slouch or experience back or shoulder pain, consider looking into a posture bra. These bras have bands that are designed to carry your chest’s weight, reducing pull on the shoulder straps and allowing you to sit or stand straight and without strain.

Do you carry a heavy bag or purse with you? If you’re constantly carrying a heavy weight on one side of your body, you are also constantly shifting to the side to compensate, which can lead to back pain and even nerve trauma. Take some time to clean out your bag, and try to alternate the side you carry it on to reduce the damage.

Correcting the Damage

Standing desks or stand-up desks are not a new fad. They’ve been around since at least the time of Benjamin Franklin, the founding father who used one over two hundred years ago. However, there remains a great deal of controversy regarding the benefits and drawbacks of stand-up desks.

A number of sources agree that standing up while you work gives you more energy and keeps you more alert. The University of Chester performed a study in 2013 that showed heartbeats rose by ten beats per minute because of standing. This increased the number of calories burned each day. In addition, blood glucose levels after lunch returned to normal far faster in those study subjects who stood as they worked.

Others have found that standing helps reduce lower back pain. Users of stand-up desks found that they engaged more fully with their colleagues and felt more ready for action if something called them away from their desk. They felt their minds wandered far less and they stayed more focused. Some users even found that stand-up desks lent themselves to certain “power poses” that benefited physiology, increasing testosterone and decreasing cortisol, the stress hormone.

Stand-up desk users seemed to agree, though, that leg and foot soreness can be a problem. This may be a particular problem for individuals who are just starting to use a stand-up desk user and whose bodies aren’t yet accustomed to the new way of working. More comfortable shoes and an anti-fatigue mat can help reduce this problem. Gradually building up the amount of time spent at a stand-up desk can also help.

If you do opt for a stand-up desk, it’s important to get one at the right height. Because each person is different, getting a custom desk built can be prohibitively expensive. A better alternative is to get an adjustable desk. One with hydraulic power can be lowered for occasional sitting and increased to a custom height to suit your individual needs.

Sitting for long periods each day can lead to all manner of illnesses—heart disease, diabetes, vein disease and more. But standing for long periods can also create health risks, especially vein disease.

A Cornell University ergonomics team found that the real solution was simply to move around regularly. If you’re sitting at a desk, stand up every 20–30 minutes and move around for two minutes. The movement gets the blood pumping, increases calorie burn, and decreases the risks for heart disease, diabetes and other ailments. This doesn’t require vigorous exercise. Simply pacing for a couple of minutes will be enough to undo the damage of sitting for half an hour. This, of course, requires an awareness of the time and a measure of discipline to move when the appointed time arrives. Adding some moderate exercise to your daily routine can do wonders for your health, even without the use of a stand-up desk.
